Laparoscopic Gastric Banding

Laparoscopic Adjustable Gastric Banding (LAGB):

Procedure

In this procedure a silastic band is placed on the uppermost part of stomach, just below the junction of esophagus (food pipe) with stomach. In this way a Stomach pouch of capacity of 15ml is created. A very small opening in the stomach pouch connects it to the remaining stomach.

Effects

This is a restrictive type of procedure. The band divides the stomach into hourglass shape. Patients fills full immediately even after eating a very little amount of food. Also, food is stored for long time as the pouch has a small opening distally. This effect gives patient satiety for longer duration.

Indications for LAGB

Patient with BMI of 33 kg/m2 with at least one co morbidity
Patient with BMI of 37 kg/m2
Excess weight of 30kgs for at least 5 years
Failure of previous attempts of maintained  weight loss with supervised methods like diet and exercise
Large volume eaters
Less than 50 years of age
Ready to exercise and accept the dietary modifications after surgery

Advantages

Least invasive among the other bariatric procedures
Technically a reversible procedure ( Removal of band causes weight gain)
Good % of excess weight loss (EWL) (40-50 %)
Does not create any major disturbance in anatomy as in other bariatric procedures
Ideal for young patient who does not want to undergo a Major (Malabsortive) procedure and is ready to exercise.
Definite improvements in associated co-morbities

Possible Complications

Immediate
  - Anesthesia complication
- Bleeding
- Injury to stomach and surrounding structures
- Pulmonary complications
- Wound infection
Late
  - Band or Port Site infection
- Band slippage
- Band Erosion
- Gastric pouch and lower esophageal  dilatation

Post operative Care

The dietary changes in post operative period will be as follows
Step One
  - Day 1 – Clear liquid diet
Step Two
  - Day 2 to Day 15 – Full liquid diet
Step  Three
  -  Day 16 to Day 30 – Semi Solid diet
Step Four
  - Day 31 onwards – Regular Diet
