Routine Health Check Up has been designed keeping in mind the needs of a common man. Everyone above the age of 35 should ideally undergo this check up annually.

| INSTRUCTIONS FOR HEATH CHECK UP CUSTOMERS |

Kindly take appointment one day prior to undergoing the Health Check Up. |

You have to observe fasting for 10-12 hours prior to undergoing the health check up. So have your dinner by 8:30 pm and after that other than water do not have anything. |

Alcohol should not be consumed on the day prior to undergoing the health check up. |
| • |
Kindly collect your first urine and stool sample in the container provided and bring it a long with you. |
| • |
Kindly bring all your medicines, medical records and your case file in case you are undergoing any treatment at present or are taking any medicine regularly. |
